The Pedometer Challenge Contest

This Pedometer Challenge Contest (“Contest”) is subject to the following rules (“Rules”)
The contest is operated by the Regional Municipality of Peel (“Region of Peel”) through Region of Peel Public Health, and will consist of a draw from among the qualified entries received by region of Peel-Public Health.
The contest period commences May 11, 2009 and runs until June 7, 2009 at 12:00 p.m. Eastern Daylight Time (the “Contest Period”)
To Participate
- If you have a pedometer, use the pedometer included in the High5Crew student package sent to grade 5 students to keep track of the total number of steps taken every day. Enter the total number of steps taken daily on the High5Crew online tracking tool. Register to use the online tracking tool at www.high5crew.ca
OR
- If you don’t have a pedometer, enter daily activities on the High5Crew online “Your Activities” tracking tool. Register to use the online "Your Activities" tracking tool at www.high5crew.ca
OR
- If you don’t have access to a computer or email, complete the log in your High5Crew booklet tracking tool. Enter your steps OR activity into the High5Crew booklet, complete and mail or drop off to:

All entries must be postmarked by June 12, 2009 and must include a signed parent permission form.
Prize
If daily steps or activities are entered on the tracking tool for everyday between and including, May 11, 2009 - June 7, 2009, the entrants name will be automatically entered to win one of the following prizes:
- Family pass to an amusement park
- Ipod
- Sports Packages
- Bike
All entries become the property of Region of Peel-Public Health. No substitutions, transfers, assignments or cash redemption of the prize is permitted. Some restrictions apply.
Conditions of Eligibility
The Contest is open to all qualified entrants who, on the date of their entry in the Contest, are residents of Peel Region, are in Grade 5 (this may include Grade 4 and/or 6 students in split classes) and attend school in Peel Region. Children of employees of the Region of Peel and/or persons living in the same household are not eligible.
Odds
Odds of winning the prize depends on the number of qualified entries received during the contest period.
Selection of the Winner
The potential winners will be selected on June 15, 2009, in a random draw from qualified entries received. A representative from the Region of Peel – Public Health will conduct the prize drawing. The decision is final. The winners will be advised on June 17, 2009. The winning entries are subject to fulfillment of all the conditions described in the Rules.
To win a prize, the potential winner must meet all eligibility requirements including the execution and return of all necessary releases. If a potential winning entrant cannot be reached by the end of 10 (ten) business days following the selection, or has not, or does not, comply with the Rules then another potentially winning entrant will be chosen from the entries received during the Contest period. The winner should receive the prize within 2 weeks of confirmation that all required documentation has been returned.

The Name the Monster Contest

To Participate
- During the contest period, log onto Region of Peel’s High5Crew website www.high5crew.ca and visit the Name the Monster Contest webpage. You can submit your name for the monster by filling out the entry form on the website.
OR
- If you don’t have a pedometer, enter daily activities on the High5Crew online “Your Activities” tracking tool. Register to use the online "Your Activities" tracking tool at www.high5crew.ca
OR
- Complete your entry for the Name the Monster Contest in your High5Crew booklet and mail or drop off to:

All entries must be submitted or postmarked by May 31st, 2009. Include in your submission your: a) name, b) school, c) home address, d) home phone number, and e) selected entry name for the High5Crew Monster.
Prize
If the submitted name for the monster meets all the listed requirements herein the Name the Monster Contest Rules and Regulations, the entrants name will be automatically entered for a chance to win the following prize:

All entries become the property of Region of Peel-Public Health. No substitutions, transfers, assignments or cash redemption of the prize is permitted. Some restrictions apply.
Conditions of Eligibility

The 1 (one) winning entry will be selected by a representative from the Region of Peel-Public Health from all eligible entries on June 2, 2009. The decision is final. The winner will be advised by June 3, 2009. The winning entry is subject to fulfillment of all the conditions described in the High5Crew Rules and Regulations. Entries will be judged according to the following criteria:
The High5Crew Monster’s name…
- Shows how you feel about being physically active,
- Includes a word related to physical activity,
- Is fun, creative, original and cool,
- Reflects the High5Crew Pedometer Challenge,
- Is not a copyright or trademark name
- Is not more than 3 words or 25 characters long.
To win the prize, the potential winner must meet all eligibility requirements including the execution and return of all necessary releases. If a potential winning entrant cannot be reached by the end of 10 (ten) business days following the selection, or has not, or does not, comply with the Rules then another potentially winning entrant will be chosen from the entries received during the Contest period. This Quiz Contest (“Contest”) is subject to the following rules (“Rules”).
The Contest
The Contest is operated by the Regional Municipality of Peel (“Region of Peel”) through Region of Peel Public Health, and will consist of a draw from among the qualified entries received by region of Peel-Public Health.
The Contest Period commences May 11, 2009 and runs until June 7, 2009 at 12:00 p.m. Eastern Daylight Time (the “Contest Period”).To Participate
During the Contest Period, log into the Region of Peel’s HIGH5CREW website and visit the school section of the site to participate in either or both of the quizzes found on the chalkboard. If you score 100% on the quiz an email entry form will pop-up. Once you fill out and submit the form, you will automatically be entered for the Contest. Entries by any other methods or means will not be accepted. All entries become the property of Region of Peel-Public Health.
Conditions of Eligibility
The Contest is open to all qualified entrants who, on the date of their entry in the Contest, are residents of the Region of Peel and are in grade 5 and attend school in the Region of Peel. Children of employees of the Region of Peel and/or those children living in the same household as an employee of the Region of Peel are not eligible to enter the Contest.

The potentially winning entry will be selected on June 16, 2009, in a random draw from qualified entries received. The draw will yield up to 25 potential winners for the True and False Quiz and 25 for the Multiple Choice Quiz for the prize as described below. A representative from the Region of Peel - Public Health will conduct the prize drawing. The decision is final. The winner’s parent or guardian will be advised on June 18, 2009. The winning entry is subject to fulfillment of all the conditions described in the Rules.
To win the prize, the potential winner must meet all eligibility requirements including the execution and return of all necessary releases. If a potential winning entrant cannot be contacted by the end of 10 (ten) business days following the selection or has not, or does not, comply with the Rules then another potentially winning entrant will be chosen from the entries received during the Contest Period. The winner should receive the prize within 2 weeks of confirmation that all required documentation has been returned.Prize

- By entering this Contest, entrants agree to be bound by these Rules. A printed copy of the Rules may be obtained by sending a written request to the following address:

Entrants further agree to be bound by the interpretation and decisions of the Region of Peel – Public Health which will be final and binding in all respects. - The Region of Peel - Public Health reserves the right at its sole discretion to disqualify any entrant it finds:
- To be tampering with the Contest entry process and/or the operation of the Contest;
- To be acting in violation of the Rules;
- Has submitted obscene or offensive feedback; or
- Has engaged in conduct in the Contest intending to annoy, abuse, threaten or harass any person.
- There is no purchase necessary to enter the Contest.
- Region of Peel - Public Health will not be responsible for telephone, mail, technical, network, online, electronic, computer hardware or software failures of any kind, misdirected, stolen, incomplete, late, illegible, garbled or delayed internet computer transmissions on account of technical problems, mail delays or traffic congestion on the internet or at any website, or any combination thereof, or for incomplete, lost, late, or misdirected submissions.
- If for any reason the Contest is not capable of running as planned, or if the administration, security, fairness, integrity or proper conduct of the Contest is corrupted or adversely affected, including by reason of infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond Public Health’s control, the Region of Peel - Public Health reserves the right to cancel, terminate, modify or withdraw the Contest without prior notice and without liability to Peel Health. If the Contest is terminated, Region of Peel - Public Health will give notice on the Region of Peel website (www.peelregion.ca).
- The Contest is governed by the laws of Canada and any applicable provincial and municipal laws and regulations. Void where prohibited by law.
- Failure by the winner to return any required documents within five (5) days of email delivery to the potential winner’s email address or return of prize documents or prize as undeliverable may result in disqualification and selection of an alternative winner, at the sole discretion of the Region of Peel - Public Health. The Region of Peel - Public Health will use reasonable efforts to pick an alternative winner but in the event an alternative winner cannot be reasonably determined, the prize will go un-awarded.
- Entrants release and hold harmless the Region of Peel - Public Health, the Region of Peel and their respective affiliates, subsidiaries, directors, officers, agents, employees, and all others associated with the development and execution of this Contest from any and all liability with respect to or in any way arising from this Contest and the awarding, use or misuse of the prize, including liability for personal injury or damage to property.
- By entering this Contest, an entrant grants the Region of Peel - Public Health and the Region of Peel the right to collect and use the personal information provided as part of entering the Contest as set out in these Rules.
